Nuform Health is a physician-owned telehealth clinic built around a deliberately different economic model than most online TRT providers: no monthly subscription. Patients pay a $50 consultation fee plus medication sold in multi-month kits (currently $295 per 5 months for injectable TRT). For a stable TRT patient, that works out to roughly $112–$127/month all-in in the first year, by our estimate from the published prices — materially less than membership models that charge a monthly fee regardless of treatment status.
The care model centers on a "1-on-1 care team": each patient is assigned a named provider and a dedicated health specialist for life, reachable by direct text and phone rather than a ticket-based patient portal. Reviewers on Trustpilot consistently name the same staff members (Romario, Bryson, Michael, Alec) — and having actual humans proactively check in every 4 weeks is the single most-cited reason patients say they stay.
Intake runs through in-person Quest blood draws rather than at-home finger-prick kits, which Nuform says are "typically not accurate." A physician reviews labs with the patient before any prescription is written, and repeat labs are recommended at 3–4 months.

Variants: if you only count 12 months of medication (2.4 kits, about $708), year 1 is ≈ $1,343; with T+hCG/gonadorelin ($120/mo, not re-checked) ≈ $2,075/yr; with enclomiphene ($96/mo) ≈ $1,787/yr. How billing works: Kits are one-time purchases billed in 3- to 5-month increments. Nuform states verbatim on its site: "We DO NOT do monthly prescriptions or subscription plans… to avoid gaps in treatment and monthly drop-off errors." Nothing auto-renews and no card-on-file recurring charge exists. Stopping treatment means simply not buying the next kit.

Nuform sells a "Testosterone + hCG/Gonadorelin" kit for $120/month, but the site does not specify whether the compound dispensed is hCG or gonadorelin. Ask at intake if this matters to you.
There's nothing to cancel — kits don't auto-renew, so you stop by not buying the next one. Per Nuform's refund page, the $50 consultation fee is refundable in full if your provider determines you aren't a candidate or you cancel at least 24 hours ahead, and non-refundable once the consultation is complete; its terms add that the fee stays refundable until blood work is completed for hormone therapy patients. Medications are non-refundable once ordered.
